When replacing everything did you replace everything with new OEM parts? Did you shim the ball at all? Which way did you adjust the Master Cylinder rod? Which direction?

I had VERY similar problems.
1st I tightened the master cylinder [was loose lol], it helped then adjusted the rod. That helped but eventually the problems came back.
Replaced the master cylinder, SS clutch line from master to slave, NEW OEM clutch fork and and fulcrum ball, NEW OEM TOB, NEW ACT 2600 PP and Custom Clutch = ALL PROBLEMS solved.
I think the main issue was the bad master/worn fork!!
